Rank was an intimate friend and "right-hand man" of Sigmund Freud, who called him the most brilliant of all his Viennese students. Rank was first hired by Freud in 1906, at the age of 22, as secretary to the small group of physicians who gathered for salon-type discussions with Freud, and soon joined the group himself. Freud became the father figure that Rank had never had, and Rank worked as his personal secretary.

Rank's therapy sessions with Anais Nin were some of the most influential experiences of her life, forever changing her views on women, sexuality, and culture. The two were also rumored to have had an affair.

Rank held therapy sessions with Henry Miller, and helped finance his 1934 book Tropic of Cancer.

Rank was a friend and colleague of Sandor Ferenczi, and collaborated on published scientific works with him.

Rank was a colleague of Ernest Jones, and though at first the two were friends, they later had a feud and many disagreements.
